We offer a medical evaluation in the form of a Medical History Questionnaire designed to detect medical reasons for your pain or discomfort, which will be evaluated by a licensed physician. If the evaluation results in a diagnosis exempting you from using a seat belt, for a $49.95 medical evaluation fee, written verification in the form of a letter and prescription form will be mailed by our licensed physician. This letter, or the prescription form, when shown to a law officer will prevent an otherwise costly citation.

Then indicate the form of payment (VISA, MasterCard, or PayPal) you wish to use for the $49.95 medical evaluation fee (a fee is charged only if the doctor determines that you qualify).
