  • All of the 49 states with seat belt laws have medical exemptions to wearing seatbelts.
    The exemption is based on a licensed physician's written statement that a seat belt cannot be tolerated for a specific medical reason after a medical evaluation. The most common reason is a temporary orthopedic problem after surgery or other injury. In this case, your surgeon or physician can supply the medical exemption.
  • What is the Medical History Questionnaire designed to detect?
    Less common than orthopedic problems, but still a valid medical exemption is related to symptoms that occur in conjunction with a condition known as sensory neuropathy. There are many causes of sensory neuropathy, some of them quite common. Symptoms associated with sensory neuropathy may be permanent - pain occurring when touched, a tingling or electric shock sensation, waxing and waning out of the blue, or an uncomfortably hypersensitive response to an ordinary stimulation. These symptoms, when caused by wearing a seat belt could result in a dangerous distraction when driving, and are sufficient to exempt a driver from the seat belt law.
  • What will I receive?
    If the evaluation results in a diagnosis of sensory neuropathy exempting you from using a seat belt, a signed letter written on the MD’s letterhead will be prepared. The letter will state that you, the patient, are seat belt intolerant and, therefore, exempted from wearing seatbelts. Also a laminated signed prescription form with an appropriate message will be included for your convenience to store on your visor or in your glove box. These will show the doctor's name, practice address, state license number, BNDD number, and American Board of Internal Medicine certificate number.
  • How long is the prescription valid for?
    The prescription will be valid for a period of 12 months. (Most states require periodic reexamination.) We will offer reexamination at the end of 12 months for an inexpensive $29.95. You can earn a renewal discount of $10 for each referral.
  • How long do I need to wait before receiving my medical exemption?
    It will generally take 7-10 business days. After approval of a credit card charge and successful medical evaluation of the questionnaire, the documents will be prepared and sent by USPS usually within 5-7 business days. (On rare occasions that the doctor is unavailable, the actual delay will be communicated via email). Of course the delay after mailing is unpredictable. 48 hour evaluation and email of a PDF copy of the exemption letter are available for an additional charge of $25. If we don't meet the 48 hour time, the $25 will not be charged.
  • Do the medical exemptions vary by state?
    All states have medical exemptions. A few states require the medical exemption to be submitted to the Department of Transportation who issues a certificate. If a certificate is required, we will sign the additional state forms at no extra charge. The exemption we issue is legally valid in the following states: AL, AK, AZ, AR, CA, CO, CT, DE, DC, FL, GA, HI, ID, IL, IN, KS, KY, LA, ME, MD, MA, MI, MN, MS, MO, MT, NE, NV, NJ, NM, NY, NC, ND, OH, OK, PA, RI, SC, SD, TN, TX, UT, VT, VA, WA, WV, WI, WY.
  • Can you be more specific about how the law reads?
    Most states, including Washington, Michigan, Florida, and Texas have similar seat belt laws. For example, the Revised Code of Washington, Title 46, Chapter 61.688 Washington's seat belt law states:
    Section (3) "Every person 16 years of age or older operating or riding in a motor vehicle shall wear the safety belt assembly in a properly adjusted and securely fastened manner."
    Section (4) "No person may operate a motor vehicle unless all passengers under the age of sixteen years are either wearing a safety belt assembly or are securely fastened into an approved child restraint device."
    Section (8) "This section does not apply to an operator or passenger who possesses written verification from a licensed physician that the operator or passenger is unable to wear a safety belt for physical or medical reasons."
  • Do you guarantee that the police will accept the exemption?
    Your diagnosis is based on your responses on the Medical History Questionnaire. Other than these questions, there are virtually no tests that the state will require to confirm the diagnosis. If, for any reason, the exemption is not accepted, we will refund all our fees.
  • Who are we?

  • Who is Dr. Buckleoff?
    Dr. Buckleoff (really Bob) is a duly licensed Washington physician who was on the medical faculty of the University of California San Francisco Medical School for 27 years, and currently is in private practice. He is board certified by the American Board of Internal Medicine.
  • Privacy Policy.
    Your right to privacy is very important to us. We recognize that providing us with personal information is an act of trust. Therefore, we have created a privacy policy to protect your personal information.
    1. Personal Information Collected
      When you visit our Web site we ask you to provide only the personal information required by us to identify you with your Personal Medical History Questionnaire.
    2. Why Driver's License Number, Phone, and Email?
      Some states require the Driver’s License Number to appear on the exemption. The other information is to simplify contacting you, should it be required.
    3. Use of Information
      We will not provide any of your personal information to other organizations or third parties unless required to do so by law. We do not sell our clients' personal information to anyone.
    4. Security of Your Credit Card Information
      We will take appropriate steps to protect your privacy. We will also protect your personal information in storage. If you supply us with your credit card information, we will encrypt the card number during transmission. Credit card information is sent directly to our Merchant Services Provider and is never seen or stored by us.
    5. E-Mail
      You may decide to send us personally identifying information, such as a message containing your name or address. We will only use this information to identify you as a client and to determine how to respond to your message. We will not use this information for any purpose other than to resolve the matter identified in your message.
